FAMILY HEALTH PLANS ONLY
If choosing the Family Health plan, payment of services will only be made under the "Primary Adult" household member's consultation form. The Primary Adult's form will be used as the official household's form time stamp, if all forms for every household member being tested are received within 2 hours of the main adult's form submission. If all household members' consultation forms are not received within 2 hours of the primary adult's form submission, then the date/time submission of the last household member's form will be used.
SCHEDULING
We will schedule you according to availability. You will receive an email with a proposed schedule date and time of service within 24 hours of booking. If you are unavailable on the proposed date/time, then we will attempt to book you at the next available date and time. We book each client in 2-hour windows. Testings are booked 8:00 am - 6:00 pm.
Note: You must fill out one form per household member. Children must be receive testing at the same time and address as the adult.
